When you go to a coffee shop, you naturally expect the barista who works there to make your coffee. You wouldn't expect someone else in a different coffee shop to make it, and bring it to the shop where you're paying for it. That would just be strange. The same goes for a hospital. If you arrive with a broken collarbone, you expect the staff who look after you will be the staff who work there! How would you react if they brought staff from a local medical centre to work in the hospital? That might seem a bit strange too. You'd want to know why!
